Below is the 2023 Team Midwest Fee Payment Registration. This is for Players and Alternates who have been selected for the 2011 and 2012 birth year teams.
Rostered Player Fee: $1,200
Alternate Player Fee: $400
You will need your Players 2023-2024 USA Hockey Confirmation #, if you don't have it you can obtain it at: https://membership.usahockey.com/
